We report a case of macular hole with ruptured retinal arterial macro aneurysm. 84-year-old female had decreased vision 2 days ago. With fundus examination, dense preretinal and subretinal hemorrhage with ruptured retinal arterial macro aneurysm was detected, we planned vitrectomy for preretinal hemorrhage. After core vitrectomy, ICG assisted ILM peeling was done. After dense subILM hemorrhage was removed. MH was detected incidentally. With C3F8 gas and face down position, macular hole was sealed successfully. If macular hole is suspected in patients with ruptured retinal arterial macro aneurysm, early operation should be recommended.
