Purpose:

To assess the patency of the superior ciliary sulcus in aphakic silicon-filled eyes by Ultrasound Biomicroscopy (UBM).

Setting:

Department of Ophthalmology, Cairo University Hospitals, Cairo, Egypt.

Patients and methods:

Ten aphakic silicone-filled eyes of ten patients were evaluated using UBM. The UBM examination of the anterior segment was performed using the Humphrey Instruments Ultrasound Biomicroscope equipped with a high-frequency (50-mHz) high-resolution¬†(50-micron) transducer. Special attention was made to the ciliary sulcus at 12 o’clock.

Results:

The UBM demonstrated intact curved aqueous-silicon oil interface which appeared at some distance from and not filling the ciliary sulcus. A patent superior ciliary sulcus was thus found in all patients. In one patient the superior ciliary sulcus remained opened despite posteriorly displaced iris diaphragm by silicone oil in the anterior chamber.

Conclusion:

The superior ciliary sulcus maintained its patency in aphakic silicon-filled eyes. The need for a 6 o’clock peripheral iridectomy should be reconsidered in these eyes.

Take-home Message:

Superior peripheral iridectomy may be suitable for aphakic silicone filled eyes to prevent pupillary block.
