Purpose:

To better understand the natural history of diabetic macular edema (DME).

Methods:

Literature review and personal experience on more than 40 patients followed for six months or more without any therapeutic intervention.

Results:

DME may show stability or even improvement of visual function and OCT morphology without any ocular therapy, especially if associated with good glycemic control.

Conclusion:

DME  is a “fluctuating” condition, tightly related to glycemic control. A correct evaluation of any therapeutic intervention needs to be related to natural history.

Take-home message:

DME may demonstrate natural stability or even natural improvement, sometimes wrongly related to therapeutic intervention.
