Surprise, Surprise. It is Sub ILM



16 Year old boy presented with sudden loss vision in the right eye one month earlier. He denied any history of trauma. The vision was perception of light. The anterior segment was quiet. The vitreous was turbid with yellowish discolouration. An initial diagnosis of posterior uveitis with severe retinitis. Pars plana vitrectomy was performed and after removal of the turbid vitreous a macular white lesion was identified. Surgery on the macular revealed a sub ILM organised haematoma. The ILM was peeled and the organised blood was aspirated exposing an intact macula and fovea. Postoperative vision improved to 1.0 within the first week. A spontaneous sub ILM haematoma with vitreous spill may cause severe visual loss.
