This is a series of 5 case with presenting with sudden drop in vision due to a large submacular blood collection. One eye was idiopathic, two due to blunt trauma and two related to age-related macular degeneration. Parsplana vitrectomy with posterior hyaloid was performed. This was followed by sub retinal injection of 75ug of tPA under the macula. Followed by sub retinal injection of few air bubbles. The eye was then filled with air and the patient instructed to stay in the semi sitting position for twelve hours. In all eyes the blood collection resolved within one or two days with marked improvement in vision. However, the final vision was determined by the original pathology. The macular scarring in trauma and AMD determined the final visual performance. No complications were encountered in any of the operated cases and is recommended in eyes with a large hematoma under the macula.
