Vitrectomy in the surgical treatment of a retinal detachment presents a lot of advantages. However, in this case of retinal detachment the vitrectomy must be as complete as possible and its iatrogenic risk must be considered. This risk cannot be accepted in stage 0 PVR retinal detachments (except in some specific cases) where the risk of failure is almost zero. In some stage A PVR the author considers that there are also no reasons to accept this iatrogenic risk and praises the advantages of adapting specifically the surgical approach to the clinical findings.
