ABSTRACT

Purpose:

To evaluate the efficacy of micropulse laser treatment in patients with diabetic macular edema and good visual acuity to prevent worsening of vision and the need of intravitreal drugs.

Methods:

We reviewed the charts of the patients with diabetic macular edema and with visual acuity of 0.67 or better that were treated with micropulse laser (532 nm, 0.2 s, 5% duty cycle, 300 mW). Center and non-center involved macular edema was treated. Patients treated with conventional laser over the previous 6 months or with anti-VEGF drugs in the previous 3 months were excluded. We measured central macular thickness and visual acuity and recorded the number of retreatments over the follow-up period.

Results:

We collected the data from 8 patients with macular edema and good visual acuity treated with micropulse laser during the year 2016. Pretreatment visual acuity was 0.86 (mean, logMAR 0.07) and central macular thickness was 279 microns (mean). Seven patients were naïve patients and one of them had received conventional laser before. After the follow-up period (mean 7.25 months) the patients presented a visual acuity of 0.89 (mean, logMAR 0.05) and central macular thickness was 268 microns (mean). The visual acuity remained the same in 4 patients, all of them had 1.0 visual acuity. Visual acuity improved in 3 patients and only got worse in 1 patient (from 1.0 to 0.83). The number of retreatments was 0.75 (mean), the number of retreatments increases with the follow-up. Only one patient got an intravitreal injection of aflibercept, after 8 months of the first laser treatment due to worsening of central macular edema, although the visual acuity remained good (0.72).

Conclusions:

Micropulse laser is a effective treatment in patients with macular edema and good visual acuity to prevent visual loss and the need of intravitreal injections.
