A myopic foveoschisis case 78 year-old high myopic female (-13.00D) with foveoschisis. This video demonstrates the unexpected iatrogenic retinal hemorrhage caused by ILM forceps trauma. Considering long eyeball length of highly myopic eye, the actual forceps length is too short for 25G plus system developed by Alcon. When trying to approach to the macula with 25G plus ILM forceps for high myopic eye, we should be careful of abrupt insertion of forceps, which might cause retinal hemorrhage or tear. In this case, the retinal arterial hemorrhage was not controlled easily, so Silicon tamponades was performed. After removing silicone oil, the foveal anatomy recovered well with good VA.
