Pathologic myopic eyes are proned to have choroidal effusions during ocular surgeries. This case had macular hole related retinal detachment who had a previous vitrectomy without success. She was offered to have a macular buckling surgery and vitrectomy. A limited subretinal hemorrhage was noted at the beginning of the surgery. We stopped at that moment and observed the choroidal hemorrhage for 10 minutes. Since it did not tend to enlarge, we decided to continue to the surgery.
