Swept-source OCT is based on a longer and tunable laser wave-lenght (1050 nm) compared to the shorter wavelenght (840 nm) spectral-domain OCT thus allowing less tailing off in detection sensitivity with depth and higher imaging speed up to 100k A-scans/s, for a better high-resolution imaging of the entire posteriore pole from the vitreous through the sclera. Furthermore motion contrast measure using a ratio method, named OCTARA (OCT Angiography Ratio Analysis), where the full-spectrum is kept intact and the axial resolution is preserved, has been provided by Topcon to obtain OCT angiography. Swept-source OCT angiography is changing the way we image vitreous, retinal and choroidal disease. In this presentation it will be showed posterior pole structures both normal and during pathology. Anatomy of the vitreous (borsa premaculare, area of Martegiani, Cloquet channel and papilla of Bergmeister) is clear and straightforward. Many informations can be derived from the vitreoretinal interface analysis with a better differentiation between vitreo-retinal adherence or traction, epiretinal membrane and pseudo-, lamellar- or full-thickness macular holes. Retinal layers as well as superficial and deep capillary plexus are imaged as never before. Vascular disease of the retina like arterial or venous occlusive disease or diabetic retinopathy can better delieneate, particularly in a early and subclinical stage (paracentral acute middle maculopathy, enlargement of the FAZ in deep capillary plexus…). New insights in the pathogenesis of rare degenerative maculopathy like unilateral acute idiopathic maculopathy will be showed. New information on choroidal neovascularization secondary to age-related macular degeneration, myopia, central serous chorioretinopathy and other degenerative maculopathy will be presented. Particular attention will be focused on explaining how OCT-angiography will help in the decision to retreat with anti-vegf drugs when the macula is still dry. Furthermore more information on choroid will be obtained during central serous chorioretinopathy. Correlation between dilated choroid will be made between OCT angi ography and indocyanine green angiography. Choroidal nevi imaging will be showed for the first time using swept-source OCT angiography and its features will impress all the audience.
