Eye’s Injury by Paintball


ABSTRACT

Diagnosis:

Right eye’s (RE) injury by paintball, during the weekend. The patient arrived at the emergence room with a lot of pain and vision of light perception in RE. He was immediately submitted to surgery and the vision and the pain improved, but not enough.

Medical History:

F.A.F, male, 21 years old, eight months ago suffered right eye injury by paintball, as soon as he removed his protection mask player

the ocular globe suffered severe trauma that cause horizontal corneal, sclera and lens rupture. it was a very extensive rupture from limbo to limbo.

The patient was immediately submitterd to surgery.

The emergence surgery was consist of scleral and corneal suture, aspiration of lens cortex in the anterior chamber, and after that, was injected subconjunctival antibiotic (vancomicin and amicacin) and an occlusive eye patch was done.

Treatment:

The patient was stable, but eight months later, the right eye presented low vision after posterior synechia, crystalline remains and fibrotic material in anterior chamber. the vision of the retina and optic disc were impossible and another surgery was performed.

On this next stage , the video of the surgery shows the removal of the residual crystalline material and synechia, capsule remains mixed with fibrin was leaved to support the intraocular lens, what was placed. In sequence, anterior chamber was closed and 23 g pars plana trocars were placed to perform posterior vitrectomy to remove the opaque vitreous and opened the center of the posterior capsule to leave the optic center free and see the retina ,and optic disc which were in good condition.

At the end, trocars were removed, antibiotic subconjunctival (vancomicin and amicacin) and occlusive dressing were done and the surgery was well succeed.

Conclusions:

postoperative visual result 20/40, satisfied patient and minimally invasive and well succeed surgery, despite having been a severe trauma with prognostic not such satisfactory as was hoped.
