We present a case of loss of the anterior chamber, forward projection of intraocular contents during placing the second suture of a corneal graft. Immediate management including forced placement of the second suture, an immediate pars plana stab with core vitrectomy, further deepening of the anterior chamber and placement of closing sutures. Postoperatively, there was recovering of vision with full function of the graft. Immediate maneuvers during critical moments in surgery are important to save the eye.
