We present the case of a 37-year-old male with an iris-ciliary body melanoma. 2 years after brachytherapy, the tumor started to grow and we decided to perform a partial lamellar sclerouvectomy plus iridectomy, followed by phacoemulsification without intraocular lens and 23 g vitrectomy for the consequent retinal detachment, leaving 5000 cs silicone oil.
7 months later, because of extreme photophobia with intense anxiety and despite presenting with macular edema plus subfoveal pigment epithelial detachment, silicone oil was removed, ultraviolet-filtering intraocular lens was sutured into the ciliary sulcus and pupilloplasty was performed. Visual acuity after 3 intravitreal implants of dexamethasone (the last 2 ones through the iridectomy) reached 0.5 with astigmatism correction at the end of a five-year follow-up.
