Lens subluxation after contusion trauma to right eye in a 67 years old man. An extracapsular cataract extraction was done and patient was refered to vitreoretinal surgery department to perform a secondary intraocular lens (IOL) implantation. Preoperative best-corrected visual acuity was 20/80 with a plus 10 dioptres spectacle correction. He complained of photophobia because of post-traumatic mydriasis. A scleral-fixated artificial iris-IOL from Opthec was subsequently implanted. We performed a knotless technique using a zigzag shaped intrascleral suture (Z suture). This knotless approach reduces suture-related complications such as suture erosion and related inflammation or infection.
