Analysis of the Macular Cystic Changes in Optical Coherent Tomography by Hee Seong Yoon, Sue Hey Chae, Hye Min Jeon, Min Soo Lee, Soh Eun Ahn, Sang Won Kim, South Korea


ABSTRACT

Purpose:

To evaluate the occurrence of macular cystic changes on spectral domain optical coherence tomography (SD-OCT) in eyes treated with pars plana vitrectomy (PPV), membrane peeling combined phacoemulsification with posterior chamber intraocular lens (IOL) insertion for idiopathic epiretinal membrane (ERM) with cataract.

Materials & Methods:

Patients with idiopathic ERM with cataract without preoperative retinal cystic changes undergoing PPV with membrane peeling and cataract surgery were retrospectively evaluated. All included patients were evaluated at 1 month following surgery, and postoperatively at 2-3 months intervals for a minimum of 2 follow-up examinations. Patients developing a characteristic macular cystic change after the surgery were analyzed and divided into three subgroups according to anatomic findings on SD-OCT: 1. the INL cystic change, 2. the CME, 3. the INL cystic change with the CME combined. The three different groups were compared regarding preoperative characteristics such as age, gender, pattern of ERM on SD-OCT, changes in the best corrected visual acuity (BCVA) of pre and post operation, and anatomic changes of the cystic lesion on SD-OCT.

Results:

The macular cystic change appeared in 45 of 551 eyes (8.1%). The INL cystic changes appeared in 27 (4.9%) eyes at a mean postoperative time period of 2.37 months. CME was seen in 9 (1.63%) eyes at a mean postoperative time period of 2.44 months. INL cystic changes combined with CME appeared in 9 (1.63%) eyes at a mean postoperative time period of 2.89 months. BCVA when the lesion first found on the SD-OCT image was the lowest in the combined group. The last observed BCVA after the surgery was also the lowest in the combined group. 19 of 27 (70%) eyes with INL cystic changes recovered at a mean postoperative time period of 4.63 months. 9 of 9 (100%) eyes with CME recovered at a mean postoperative time period of 5.11 months. 7 of 9 (78%) eyes with combined lesion recovered at a mean postoperative time period of 5.11 months.

Conclusions:

A mean occurrence of macular cystic changes undergoing PPV and membrane peeling combined phacoemulsification with intraocular lens insertion was about 8.1%. The INL cystic change was the most common (4.9%) and BCVA after the surgery found the lowest in eyes with combined INL cystic change and CME.
