After several IVTs on a macular hemorrhage, after 2 months, a huge and calcified hematoma with a complete loss of vision was successfully operated with a trans-retinal extraction of the hematoma. The removal of the hematoma in the vitreous cavity was possible thanks to the use of an aspiration-flow control pump. The main secret of this surgery is to remove by aspiration all the blood above and under the retina. The surgery was ended by a liquid to silicone exchange after laser.

Take home message:

This desperate case could have been avoided by an earlier surgery.
