Purpose:

To report a 29 gauge endoillumination assisted scleral buckling technique for rhegmatogenous retinal detachment (RRD) using a new illuminated indenter-marker and non-contact wide-angle viewing system.

Methods:

Nine consecutive cases with RRD underwent 29 gauge endoillumination assisted scleral buckling using a new instrument, illuminated indenter-marker and non-contact wide-angle viewing system. The eyes were anaesthetised using sub-tenon anaesthesia. In 5 case the drainage of the sub retinal fluid was performed. The extension of the buckling area, and pain during the surgery were assessed in each case. The duration of the modified technique was measured and compared to the duration of regular scleral buckling procedure performed by the same surgeon. Cryoretinopexy was substituted with indirect laser photocoagulation.

Results:

The mean best corrected visual acuity (BCVA) of the study eyes was 20/200 preoperatively. The retina reattached in 9 cases of 9 eyes (100%). The mean BCVA at the second day after the surgery was 20/80. The mean surgical time of the procedure was 33.8 minutes ± 4.12 minutes. In all cases a minimal segmental buckles were used supporting only the targeted area where the retinal holes were located.

Conclusion:

Small gauge endoillumination assisted scleral buckling is a novel approach to old fashion surgical procedure for RD. The implementation of endoillumination allows to perform the procedure using the microscope and non-contact wide-angle viewing system. The use of a new instrument, illuminated indenter-marker facilitates easier, more convenient, controlled and precise marking of the desired area during the indentation and localisation of the retinal breaks. Because the illuminated indenter-marker allowed to localise the retinal holes with minor indentation, the sub-tenon anaesthesia facilitated the painless procedure. A case selection has to be considered.
