SYNOPSIS

25-g TSV allows a faster patient’s recovery but sclerotomies can leak. We treat a myopic MH with RD where extensive vitrectomy with gas endotamponade is required, therefore we perform new tunneled sclerotomies parallel to the scleral fibers. We use TA to highlight the vitreous, ICG to stain the ILM and long-acting gas endo-tamponade. After the trocars removal, the sclerotomies are completely airtight and immediately self-seal. Therefore 25-g TVS may be safely performed in myopic MH with RD.
